All sub-recipients are required to have Clinical Quality Management Plans and Annual Work Plans. The link below includes a checklist that is used when reviewing sub-recipient plans and a template that can both be used to create plans and work plans. Additional documents are included to provide guidance and tools for creating and maintaining a robust CQM program.
