Weekly Mailing

United Way of Long Island regularly sends out a weekly mailing of HIV related information as a resource for both providers and consumers within the community.

Needs Assessment & Survey Reports

Consumer Survey - The Nassau-Suffolk HIV Health Services Planning Council is responsible for determining HIV service needs on Long Island and establishing priorities for the allocation of funds. The Consumer Needs Assessment Survey is one of the tools the Council uses to help determine service needs and gaps for people with HIV (PWH) on Long Island.

Integrated Plan - New York State’s Integrated HIV Prevention and Care Plan  is a comprehensive strategy focused on HIV prevention and care in New York State.

Provider Survey Report - The N-S EMA Provider Survey gathers information from sub-recipients (funded providers) about HIV-related services, client needs, barriers to care, training needs, and challenges providers face in providing care.

Community Forums

Each year, the Nassau-Suffolk HIV Health Services Planning Council engages in the Priority Setting and Resource Allocation (PSRA). Data collection is a vital part of the PSRA process and helps to determine which priorities will be funded and how much funding will be requested by the region as part of its annual HRSA grant submission request. A major component of the data that is collected includes information gathered directly from the public during annual community forums.   

Administrative Mechanism

The Ryan White Program requires the establishment of a mechanism to administer funds for the timely delivery of essential services to PLWH throughout Long Island. This mechanism is used to allocate funds according to the Planning Council’s priorities. The assessment of the administrative mechanism is done annually and is a roadmap for what was done well and to identify areas for improvement.

Ryan White Data

The Ryan White program provides HIV services throughout the United States; more than half the people with HIV in the U.S. get services under Ryan White. New York State publishes HIV surveillance data reports every year and the Nassau-Suffolk EMA compiles data for the region.

Council Documents & Forms

Bylaws - Bylaws are the internal rules established by the Nassau-Suffolk HIV Health Services Planning Council to govern its operations. They define the structure, roles, and responsibilities of the Council, and detail how meetings are held, voting procedures, and other internal processes.

Grievance Procedures - Any individual affected by a Planning Council process may submit a grievance to the Planning Council. The grievance procedures describe the process for a formal grievance, mediation and arbitration and includes the documents needed to file a complaint.

Confidentiality - The confidentiality policy explains that no confidential information will be disclosed or used to further financial or other private interests except to the extent that disclosure is necessary to fulfill a responsibility or is required by law, including any law that requires meetings be held in a public setting. 

Conflict of Interest - The conflict of interest statement is signed by Planning Council and committee members to ensure that they will not vote on any matter or person with which they have a real or perceived conflict of interest.
